Is it possible to correct breast asymmetry with identical implants?

Breast asymmetry, when one breast is different from the other, is relatively common. Nobody has one side exactly like the other. Small differences are acceptable and extremely common. In breast surgeries, this is another challenge! We, plastic surgeons, seek the best way to make these breasts similar. Before that, we have to identify and characterize these differences. Many times, they are related to posture or chest wall deformities. But how to correct these deformities? We use techniques commonly used in cosmetic surgery to decrease or increase volume, such as augmentation mammoplasty with silicone implants or reduction mammoplasty by resecting tissue. This way, we adjust them to make the breasts more symmetrical. It is common in these cases for doctors to use silicone implants of different volumes. This solution can bring other complications. For example: breasts with different implants will never be identical, and over time, with weight gain or loss, breastfeeding, or pregnancy, this difference can become increasingly evident. Therefore, for cases with small differences in breast volume, I prefer to use identical implants, and why? Imagine: one breast with 80 ml and another with 100 ml; if we place a 300 ml implant, one will be 380 ml and the other 400 ml. In the end, this difference will change from 20% to 5% of the final volume. This way, I end up correcting by the proportion of the final volume, without having to use different implants with different shapes! Understand? Another situation is when a mammoplasty is necessary; tissue resection is never identical, but what needs to be more similar is what remains in the breast. The idea is the same, always leave the breast as similar as possible, try to use techniques in the most similar way... For these breast asymmetry surgeries, I also use molds during the surgery, which facilitate these choices, and during the consultation, I perform simulations with a 3D camera that can estimate the volume of each breast, facilitating this calculation.
