Lipedema, lymphedema, and cellulite: what changes and how to treat

For years confused with obesity or cellulite, lipedema gained prominence in the Brazilian press in 2025-2026. Dr. Fernando Amato was consulted by Ana Maria, Guia da Farmácia, TIM News, and Snifdoctor to differentiate lipedema, lymphedema, and cellulite, and explain when liposuction is used as a treatment.

Lipedema

A chronic disease of adipose tissue, almost exclusive to women, with a strong hormonal component. Symmetrical accumulation of fat in the legs, hips, and sometimes arms, sparing the feet and hands. Pain on palpation, sensation of heaviness, and spontaneous bruising.

Lymphedema

Accumulation of lymph due to lymphatic drainage failure. Generally asymmetrical, affecting feet and toes (positive Stemmer's sign). Common after axillary dissection for breast cancer.

Cellulite

Alteration of the architecture of the subcutaneous tissue. Only aesthetic, not painful, not progressive. Do not confuse with lipedema.

Lipedema treatment

• Elastic compression and lymphatic drainage;
• Low-impact physical activity (aquatic aerobics);
• Anti-inflammatory nutritional management;
• Specific liposuction for lipedema (WAL/PAL) in the indicated stages.
