Does silicone for scar treatment work?

Silicone gel sheets and silicone gel are known for their effectiveness in scar reduction and other semi-occlusive properties. These agents work through occlusion, which is based on impeding the chemical process of absorption, heat, or pressure. Additionally, silicone gel has the ability to decrease cytokines IL-1, TNF, IL-8, and Cox-2, among others.

The application of silicone gel to scars with a deficient epithelial water barrier due to an immature stratum corneum results in decreased water loss. This prevents an increase in sodium concentration, which would otherwise result in a Na (sodium)-mediated signal transduction pathway that includes the activation of various genes and cytokines, such as TGFB. Although other occlusive treatments are also effective, it is important to note that complete occlusion can result in excessive hydration and negative impacts on surface bacterial levels, as well as undesirable changes in the epithelium.

Duration of use for silicone gel strips

However, 12-hour use of silicone gel is effective in reducing erythema and flattening scars. However, 24-hour use can cause maceration and be poorly tolerated.

Evidence suggests that silicone gel is effective in preserving and treating immature scars, but has no effect on mature scars that have already gone through the maturation process for a year without presenting erythema.

Silicone cream should be applied at least twice a day. Several studies have shown that silicone gel significantly reduces various scar attributes, including pigmentation, vascularization, malleability, height, and symptoms of pain and itching.

In summary, the use of silicone gel strips and silicone cream has a history of almost 40 years, with widespread clinical use for about 20 years.

However, studies have found that the semi-occlusive properties of silicone gel have an attractive mechanism of action, normalizing transepidermal water loss in scars with deficient stratum corneum barrier function, through sodium channels in the cell membrane.
