CORRECTION OF DIFFERENT BREASTS WITH THE SAME PROSTHESES

Is it possible to correct breast asymmetries with identical prostheses? Breast asymmetry, when one breast is different from the other, is relatively common. No one has one side exactly equal to the other side. Small differences are acceptable, and extremely common. In breast surgeries, this is another challenge! We, plastic surgeons, look for the best way to make these breasts similar. Before that, we have to identify and characterize these differences. Many times, they are related to posture or chest wall deformities. But how to correct these deformities? We use techniques commonly used in cosmetic surgery to reduce or increase volume, such as reduction mammoplasty with a silicone implant or reduction mammoplasty by resecting tissue. This way, we adjust to make the breasts more similar. It is common in these cases for doctors to use silicone implants with different volumes. This solution can bring other complications. For example: breasts with different implants will never be identical. And over time, with weight gain or loss, breastfeeding, pregnancy, this difference can become increasingly evident. Therefore, for cases with small differences in breast volume, I prefer to use identical prostheses, and why? Imagine: one breast with 80 ml and another with 100 ml; if we place a 300 ml prosthesis, one will end up with 380 ml and the other with 400 ml. In the end, this difference will no longer be 20% but 5% of the final volume, thus correcting by the proportion of the final volume, without having to use different prostheses with different shapes! Understand? Another situation is when mammoplasty is needed, tissue resection is never identical, but what needs to be most similar is what remains in the breast. The idea is the same, to always leave the breast as similar as possible, trying to use the techniques in the most similar way… For these breast asymmetry surgeries, I also use molds during surgery, which facilitate these choices, and in the consultation, I perform simulations with a 3D camera that can estimate the volume of each breast, facilitating this calculation.
